Report the Death of a Member
In the unfortunate event of the untimely death of a WAEPA policyholder, we extend our deepest condolences to you and your family. During this challenging and overwhelming time, please know that we are committed to helping you through the claims process.
What Steps Do I Need to Take to File a Claim?
1. Start the process by completing a Death Claim Form. You’ll need the following information:
- Group Certification Information; this can be found on the insured’s Group Term Life Insurance certificate
- Deceased Information, including their Date of Death and Manner of Death
- Beneficiary Information, including their Social Security or Taxpaper Identification Number
- You will also need to provide a copy of the Certificate of Death
2. Submit your completed form through our secure Member Portal. You can also email the form to claims@waepa.org, or mail a physical copy to our WAEPA headquarters at 433 Park Avenue, Falls Church, VA, 22046.
What Happens After I File a Claim?
WAEPA is proud to be underwritten by New York Life Insurance Company. With more than 175 years of experience, you can trust New York Life to help protect the financial future of your loves ones.
Once you’ve completed your form, New York Life Insurance Company will process your benefit on behalf of WAEPA as quickly as possible. You can expect to receive your benefit 10 business days after New York Life receives your completed Claim Form, Death Certificate, and other documents.

Support Doesn’t End with a Claim
WAEPA beneficiaries have access to the Empathy program, through New York Life. This platform can provide you with a variety of resources, covering both the practical and emotional challenges that follow the loss of a loved one. These include grief support, probate guidance, family collaboration, and access to care experts.
